Preparing for the appointment

Each signer must provide a current (not expired) picture identity card. All forms of identification must include a photograph, a physical description, a signature, and a unique serial number. The state of Arizona accepts the following forms of identification.

Valid types of Identification

The identification card must be unexpired and issued by the federal government, a state government, or a tribal government. 

1. Driver License/State I.D.

contains the customer’s portrait, signature, and physical description, which includes the customer’s height, weight, eye color, and hair color. Issued by: A state or territory of the United States. 

2. U.S. Passport

Issued by the United States Goverment.

3. Military Identification Card

Unexpired 

4. Other Gov’t Issued Identification Cards

comprises the customer’s portrait, signature, and personal description, which includes his or her height, weight, eye and hair colors.
Issued by: The federal government; a state, a territory of the United States, or a tribal authority.

Know what you are signing

Every signer must demonstrate a willingness to sign the documents without coercion. He/she must also be aware of what they are signing and understand the significance of the transaction. Documents cannot be notarized if there is any cognitive impairment. A Notary Public is not permitted to provide legal advice by state laws.
